1. Membership is only applicable for age at 18 or above and can enjoy online appointment through our web site.

2.The date of individual appointment is limited within 2 weeks. For details about the available dates, please call us or check on our Facebook fan page.

3. No membership fee is required, but all correct information must be completed. After submission, we will reply you via email within 1-5 days when your application form has been received.
